This Wellness Snapshot is an interactive self-assessment tool built in Articulate Storyline for a 12-week guided plant-based lifestyle program. The course helps users reflect on their lifestyle habits and track progress through a dynamic before-and-after quiz format. Designed to encourage self-awareness and habit change, this module combines data capture, personalized feedback, and intuitive UI elements.

Key Features:

Likert Scale Interaction: Each question uses radio buttons and variables to simulate Likert-scale behavior within a quiz-based environment.
Custom Progress Tracker: A visual progress bar updates in real time using triggers and number variables to improve learner orientation and reduce abandonment.
Dynamic Score Capture: Each section’s score is stored in dedicated variables (slide1score to slide9score) and compiled into a final summary using calculated total score variables.
Personalized Feedback Layers: Feedback is dynamically revealed based on score thresholds, encouraging tailored learner reflection.
Email Entry with Validation: A text entry field is paired with JavaScript to verify email format before submission, ensuring accurate backend data collection.
Backend Data Capture via Google Forms: Quiz data and email inputs are passed via JavaScript to a hidden Google Form connected to Google Sheets, allowing for backend data analysis without the need for an LMS.
Optional Email Follow-Up: Learners are given the option to receive a copy of their scores and feedback via email, increasing personalization and program continuity.

I started by designing the quiz around nine key wellness areas, each with custom Likert-style questions to assess different aspects of the user’s lifestyle. Since the quiz was intended to capture a “before and after” snapshot for a 12-week plant-based program, it was essential to ensure each section had its own score tracking without overlap or data carryover.

Articulate Storyline doesn’t natively support dynamic Likert-style scoring, so I built each slide manually using radio buttons, custom states, and number variables. I introduced a temp score system with logic triggers that stored each response accurately and avoided cumulative miscalculations, especially when users navigated between slides.

Because the course wasn't hosted in an LMS, I used JavaScript to validate the user’s email address and submit results to Google Forms, which fed directly into a Google Sheet. This workaround allowed for data capture and optional progress reporting without relying on an LMS, while still maintaining a seamless user experience.

To further support the learner, I added a custom progress tracker using a slider, animated through variable triggers. This visual cue helped learners stay oriented and engaged, even as they moved through more introspective questions.
